Prime Care is a provider for the State of Indiana Medicaid Waiver Program.

These waivers allow Prime Care to provide Residential & Community assistance to Consumers:

• Community Integration and Habilitation Waiver
• Family Supports Waiver
• Traumatic Brain Injury Waiver

We are certified to provide these services under the state waivers:

• Recreational Therapy
• Personal Assistance & Care
• Residential Services
• Respite Care
• Community Habilitation
